摘要: | 妥瑞症(Tourette syndrome)發病於兒童期的慢性神經精神異常,平均發病年齡是7歲,其症狀包括動作型與聲語型的tic,且常合併發生過動、強迫、學習障礙與自殘等會影響妥瑞青少年與人際發展。這些症狀與合併症多為外顯動作對於病人生理、心理及社會層面均造成相當大的影響。妥瑞症干擾其與同學、教師的互動,且對融入團體產生障礙,雖然隨著年齡增長會試著掩飾症狀,但這些問題會讓其感受到自己的「不同」,對於無法控制自己的身體而產生焦慮、挫折、沮喪甚至於嚴重憂鬱。本研究擬以現象學研究法,以立意取樣收案,藉由深入訪談收集資料。訪談資料依循Giorgi之現象學分析、整理及歸納的結果,得到三個主題:(1) 人我互動隔閡;(2)擊鼓宣揚妥瑞但效果不彰;(3)期待與妥瑞共存。研究結果可提供給醫護人員瞭解妥瑞症青少年之生病經驗之本質結構,同時有助於醫護人員、學校教師擬定妥瑞症青少年因應疾病所帶來之身心困擾之問題的照護指引,並建立妥瑞症諮商介入措施,以提升妥瑞症青少年的照護品質。 Tourette syndrome is a chronic neuropsychiatry disorder which develops symptoms during childhood, with 7 being the average age of onset. Symptoms include motor tic and vocal tic disorder, and usually combine with the occurrence of hyperactivity, compulsions, learning disabilities, and self-injury. Since all these symptoms and combined disorders are explicit in nature, they can not only influence the development of social relationship between adolescents and others, but also bring a relatively significant impact on various aspects such as the patient’s physiology and psychology and the society. Previous study indicated that the Tourette syndrome can interfere with the interaction activity between the patients and their classmates and teachers and may produce obstacle to the patient’s involvement in group activity. Although the patient may manage to disguise these symptoms when they become older, this also tend to make them consider themselves as different from the others, and may have the problems of anxiety, frustration, disappointment, or even severe depression when finally they are unable to control themselves. The results obtained from this analysis were divided into three major themes. Change and barrier between self and others, no much positive effects from the dissemination campaign of Tourette Syndrome , and expectation of viewing Tourette Syndrome as a normal behavior. Hopefully, this study will offer healthcare provider an opportunity of deeply understanding the illness experience of adolescent with Tourette Syndrome, and at the same time can help healthcare providers and teachers establish guideline of assisting they to adapt to school life and construct the consultation intervention measures, to provide a higher quality care for adolescent with Tourette Syndrome. |
