題名: 神經科護理人員之專業承諾

摘要: 目的:探討神經科護理人員個人特性及環境因素與其專業承諾的關係。
研究方法:研究設計採橫斷式方便取樣研究法,在兩家區域教學醫院神經內外科病房與加護病房針對護理人員101名,使用結構式問卷進行資料收集;收集內容包括專業承諾的相關因素和護理工作的壓力等。
結果:神經科護理人員之專業承諾之得分在四分法量表中平均為2.78(承諾程度屬於中度偏強)。與專業承諾有顯著相關的因素為個人主觀的工作壓力感、教育程度、婚姻狀況、聘用方式(約聘或專任),整體之解釋總變異量達262.%。主觀壓力感是影響專業承諾之主要因子之一,與專業承諾呈顯著正相關,其可解釋最大的變異量達9.6%,也就是壓力愈大,專業承諾相對就愈高;其次是婚姻狀況,與專業承諾間也存在著顯著正相關,其單獨解釋力有72%;而專任護理人員之專業承諾也較高,但是教育程度的提高卻呈現專業承諾的下降。專業承諾量表的四個因素中『專業努力意願』、『專業正向評價』、『專業生涯涉入意願』均與主觀壓力成呈顯著正相關。
結論與討論:由本研究可知神經科臨床護理人員之專業承諾概況,提供醫護管理者進一步研擬提高專業承諾的介入方案,以期較高專業承諾的護理人員能維持適度工作壓力及調適能力,繼而提昇臨床照護品質。
Purpose: To understand the professional commitment of nurses in neurologic units and its relations to personal and environmental factors.
Method: A cross-sectional study with nonrandom sampling was used. we recruited 101 nurses from general or intensive care units in neurological and neurosurgical departments of two regional teaching hospitals located in the Northeastern Taiwan. Data were obtained by structured questionnaires, which included scales for professional commitments, work-related stress, and personal information.
Results: The mean score of professional commitment was 2.78, based on a 1 to 4 Liked scale, which was above moderate level. Subjective working stress, educational level, marital status and employment condition were found to have significant correlations with professional commitment. Those factors accounted for 26.2% of the total variance by regression analysis. Subjective working stress, accounting for 9.6% variance, was a major predicting factor for professional commitment. Higher level of working stress was associated with greater professional commitment. Professional commitment is also significantly different between nurses in single or married conditions. Interestingly, educational level was negatively correlated with professional commitment. Three of four subscales of professional commitment were all highly correlated with subjective working stress.
Conclusions: Professional commitment in neurological nurses is above moderate level. The high correlation between professional commitment and working stress warrants further interventional studies to depict their complex relations along with other personal and environmental factors.
