摘要: 慢性腎臟疾病(CKD)已成為全球主要公共衛生問題,影響約9.1%至13.4%的人口。在台灣,成人CKD盛行率超過12%,且多數早期病患未察覺病情。因此,及早發現和預防CKD患者的骨質疏鬆症是健康管理的重要環節。分析CKD患者的基本資料、骨質密度、骨折風險及健康狀態與FRAX骨折風險之關聯,預防CKD患者骨質疏鬆及骨折提供臨床參考。回溯收集2,400筆病歷資料,對象為GFR≤60 mL/min/1.73 m²且接受DXA評估之20歲以上CKD患者。收集其基本資料、骨質密度、骨折風險及健康狀態等信息,並分析這些因素之間的關聯性。男性之身高、體重、L-Spine T-Score及L-Spine BMD均顯著高於女性;女性的FRAX骨折率顯著高於男性。年齡與骨密度呈負相關,體重和BMI與骨密度呈正相關。影響骨折風險的顯著因子包括年齡、身高和BMI。女性骨質密度顯著低於男性,且FRAX骨折風險高於男性。建議對CKD患者進行詳細的骨質疏鬆篩檢和骨折風險評估,針對年齡、身高、體重及BMI等關鍵因素,及早識別高風險患者,實施預防和治療策略,降低骨折風險,改善生活品質。
Chronic kidney disease (CKD) affects 9.1% to 13.4% of the global population and over 12% of adults in Taiwan. Early detection and prevention of osteoporosis in CKD patients are crucial.Objective: To analyze the relationships between basic information, bone density, fracture risk, and health status with FRAX fracture risk in CKD patients. Retrospective study of 2,400 CKD patients aged 20 and above with GFR ≤ 60 mL/min/1.73 m² who underwent DXA assessments. Data on basic information, bone density, fracture risk, and health status were collected and analyzed. Males had higher height, weight, L-Spine T-Score, and L-Spine BMD than females, who had higher FRAX fracture rates. Age negatively correlated with bone density, while weight and BMI were positively correlated. Significant fracture risk factors included age, height, and BMI. Females had lower bone density and higher FRAX fracture risk than males. Detailed osteoporosis screening and fracture risk assessment are recommended for CKD patients. Early identification of high-risk patients and preventive strategies can reduce fracture risk and improve quality of life. Regular assessments should be part of CKD patient management.
