摘要: 依據健保署2020年統計調查指出,腎臟疾病的治療費用高達533億元,名列健保支出項目第一位。在台灣末期腎臟病患中,約有九成選擇以血液透析治療來延續生命,而功能良好的血管通路,是血液透析患者賴以生存的生命線。研究者依據照護血液透析病患多年經驗,深知當病患面臨血管通路問題時,其所承受生理及心理上壓力非常之重;若醫療供給者能提供一個即時通訊平台,不但能在第一時間立即為病患做通路處置的安排,不致於影響血液透析療程;更進一步可以在治療前後給予關懷及專業的衛教服務,除了能降低病患心中不安外,亦可為血液透析病患與醫療團隊間的醫療服務帶來更多益處。據此,本研究以目前使用血液透析病患通訊平台的「醫關懷App」為研究標的,並結合計畫行為理論與科技接受模式作為理論基礎,針對目前使用此應用程式之血液透析病患,採立意取樣180位,並以網路問卷調查方式進行資料收集,問卷發放時間為2022年1月至3月間,共回收173份有效問卷,整體有效回收率為96%。本研究主要結果顯示:(1) 使用者的認知易用性對於其認知有用性有正向顯著影響;(2) 使用者的認知易用性對於行為意向有正向顯著影響;(3) 使用者的認知有用性對於行為意向具有正向顯著影響。本研究對於業者的建議為:首先是加強App本身的穩定性及資訊的正確性;其次應強化資訊的易讀性與加強訊息的接受程度;此外,資訊的記錄、保存與連結也應更加確實,可為軟體設計廠商創新APP功能提供依據以及未來相關研究者之參考。
According to the formal statistical survey of the National Health Insurance Ad-ministration in 2020, the treatment cost of kidney diseases was as high as 53.3 billion NTD, which ranked at the first place among expenditure items in health insurance. About 90% of patients in Taiwan with end-stage renal disease would choose hemodialysis to prolong their lives. Therefore a well-functioning vascular access is the elements of survival for hemodialysis patients. Based on the experiences of taking care of hemodialysis patients in decades, the researcher understood the serious physical and mental pressure of the patients when they faced vascular access problems. When the medical providers could provide an instant platform to answer, it would not only make arrangements for the vascular access treatment for the patient immediately, but also not to affect the coming hemodialysis treatment courses. Furthermore, caring and professional health education services could be given before and after the treatment courses. Those courses would reduce the anxiety of the patients, and brought more benefits to the patients and the medical team as well. Accordingly, this study took the "Medical Care App", which was currently using as the communication platform for hemodialysis patients to be the research object. The study also combines the theory of planned behavior (TPB) and the technology ac-ceptance model (TAM) as the major theoretical framework. A total of 180 valid question-naires were collected, and the data was collected through an online questionnaire survey. The questionnaires were distributed from January to March 2022. A total of 173 valid questionnaires were recovered, and the overall effective recovery rate was 96%. The main results of this study showed that: (1) the user's cognitive ease of use had a positive and significant effect on their cognitive usefulness; (2) the user's cognitive ease of using had a positive and significant effect on behavioral intention; (3) the use of cognitive useful-ness had a positive and significant effect on behavioral intention. The recommendations of this study for the industry were: firstly, strengthen the stability of the APP itself and the correctness of the information; secondly, strengthen the readability of the information and enhance the acceptance of the information; in addition, the recording, storage and linking of the information should also be more reliable, which could provide a basis for the software design manufacturers to innovate APP functions and a reference for relevant researchers in the future.
