Abstract: 為了全人類的健康促進,提升母嬰照護的品質,成功的全母乳哺育及母乳哺育率的提升,配合推動發展中央政策,建構及發展協助母乳哺育支持系統人員即哺乳照服人員的專業職能,提升哺乳照顧服務人員人才素質,照顧品質,提升其競爭力,同時促進培訓產業的發展,滿足社會暨產業的需求。本研究經由相關文獻探討母乳哺育的重要性與對母嬰健康的影響、影響母乳哺育的因素、現今國際與政府之政策,與社會需求及現階段母乳哺育教育知能相關之文獻。依據研究目的整理相關文獻理論,彙整出哺乳照顧服務人員之工作內涵。並邀請專家檢核,採用修正式德菲法問卷,建構哺乳照顧服務員之職能內涵表。以「提升母嬰照護暨泌乳技能培訓班」產學合作計畫-設計以哺乳照顧服務人員工作內涵為基礎發展的職能課程,針對結訓的培訓班學員進行課滿意度相關的問卷調查,並統計課程學習前測驗與後測驗之成績,彙整與分析結果,主要在於驗證哺乳照顧服務人員專業職能內涵,以作為哺乳照顧服務人員發展職能導向課程設計時之參考。
本研究主要結?如下:一、哺乳照顧服務人員在執行哺乳照顧服務工作時,需具備5項工作任務、25項行為指標、26項知識、17項技能與6項態度之職能內涵。二、哺乳照顧服務人員職能課程知識應用滿意度,以「奶水儲存及擠奶技巧」為最高。三、哺乳照顧服務人員職能課程技能應用滿意度,以「在開始哺餵母乳前,請產婦洗手」為最高。四、哺乳照顧服務人員職能課程知識及技能應用平均滿意度,分別為93.8%及96.5%。課程前測成績平均為62分,後測成績為91分,有相當顯著的差異。依據研究結果建議:一、建構及發展協助母乳哺育支持系統人員即哺乳照服人員的專業職能,提升哺乳照服員人才素質。二、可強化哺乳照顧服務人員有關奶水儲存及擠奶技巧之專業知識應用之職能。三、可強化哺乳照顧服務人員有關在開始哺餵母乳前,請產婦洗手之專業技能應用之職能。四、持續辦理提升母嬰照護暨泌乳技能培訓班產學合作計畫,培訓具有哺乳照服專業職能的哺乳照服人員。
In order to promote the human health, improve the quality of care for mother and infant, increase exclusive breastfeeding and breastfeeding rate successfully, cooperate to press ahead with government policy, set up and develop the breastfeeding support workers and the professional competency of breastfeeding nursing personnel , improve the quality of nursing personnel talent and care quality, enhance their competitiveness, and enhance the training industry development to meet the needs of society and industry at the same time. This study explored the importance of breastfeeding and its impact on maternal and infant health, factors influencing breastfeeding, nowadays international and government policies, and social needs and current knowledge associated with breastfeeding education from literatures review. According to the research objective to review the related theories, and the job of breastfeeding nursing personnel was summarized. Experts were invited to check and constructed occupation competency table of breastfeeding nursing personnel with modified Delphi method questionnaire. Based on the industry-academic collaboration program of " training course of skills of mother and infant care and lactation improving ", a functional curriculum was designed, to develop the job content of breastfeeding nursing personnel. A satisfaction survey was conducted for those trainees who finished the training course. The results of the pre-test and post-test of the course were collected and analyzed, mainly to verify the professional occupational competency of breastfeeding nursing personnel. It can be used as a reference for the course design of functional orientation of breastfeeding nursing personnel.
The main conclusions of this study are as follows: first, breastfeeding nursing personnel should have 5 tasks, 25 behavioral indicators, 26 knowledge, 17 skills and 6 attitudes corresponded to their occupational competency. Second, the highest satisfaction to nursing staff's functional curriculum knowledge application was "milk storage and milking skills". Third, the highest satisfaction to the nursing staff's functional curriculum skills application was "asking mothers to wash their hands before breastfeeding". Forth, the average satisfaction rate of the application of knowledge and skills in functional curriculums of nursing personnel was 93.8% and 96.5%, respectively. The average score of pre-test is 62 points and the post-test is 91 points, with significant difference. According to the results of the study, the following suggestions are proposed: First of all, set up and develop the breastfeeding support workers and the professional competency of breastfeeding nursing personnel, also improving the quality of nursing personnel talent. Secondly, strengthen nursing staffs’ professional knowledge application about milk storage and milking skills. Next, strengthen nursing staffs’ professional skill on asking mothers to wash their hands before breastfeeding. Last but not least, to continue to carry out the industry-academic collaboration program of the training course of improving skills of mother and infant care and lactation to train more breastfeeding nursing personnel with professional breastfeeding and care occupation competency.
