摘要:	本研究以臺北市某個案醫院為研究對象，採取質性研究取徑的個案研究法。主要探討臺灣建構高齡友善健康照護醫院的重要性、認證標準、個案醫院完成「高齡友善健康照護醫院」認證發展策略、困境與改善作法。最後，並探知長期門診高齡者對個案醫院友善氛圍的感受。研究結果如下：一、臺灣建構高齡友善健康照護醫院有其必要性，兒童醫院、精神醫療機構及診所無迫切需要。二、高齡友善健康照護醫院認證標準以「管理政策」標準最重要，認證標準執行能提升醫療照護服務品質。三、個案醫院實踐多元化之高齡友善健康照護的策略，這些策略如下：(一)依循高齡友善健康照護機構認證標準推動醫院目標與願景；(二)老人照護流程特殊及軟硬體的設施設備客製化；（三）成立「健康醫院委員會」；(四)研擬健康促進照護流程及建置無障礙設施設備；(五)醫院認同高齡友善健康照護政策是實施高齡友善健康照護醫院的首要因素；(六)適宜的老人照護以實踐高齡友善生命關懷；四、個案醫院推行醫院認證的困境與改善策略：(一)困境：1.認證標準沒有分級與差異；2.認證標準沒有簡化；3.大部分高齡者缺乏健康識能。(二)改善策略：1.依循認證標準成立院層級委員會與跨團隊推動小組執行；2.安排高齡者可配合參加衛教課程時間、運用門診等候看診播放宣導衛教影片及跨團隊領域技巧評估列冊、追蹤與轉介衛教中心；3.建置網站資料庫以簡化資料準備與便於查詢。最後，並提出結論與建議，以為相關單位在推行高齡友善健康照護醫院之參酌。 This study took a case hospital in Taipei City as the research object and adopted a case study method of qualitative research.This paper mainly discussed the importance of age-friendly hospitals (AFHs) in Taiwan and certification standards. It also investigated the dilemma and strategy to achieve AFH certification in the case study hospital. Finally, it explored the feelings of long-term outpatients on the friendly atmosphere of the case study hospital. The results of the study were as follows: First, there was a need for Taiwan to build AFHs, while there was no urgency for children's hospitals and mental health institutions. Second, the "management policy" standard was the most important among the certification standards. The quality of medical care also improved after the certification process. Third, the case study hospital implemented various strategies of age-friendly health care:1.Create the goal and vision based on AFH certification standards.2.Customize the care procedures, hardware equipment, and software facilities for the elderly.3.Constitute a "Health Hospital Committee."4.Develop health-promotion care processes and build a barrier-free environment. 5.Heighten the staff's agreement with the importance of age-friendly care policy for the AFH implementation. 6.Humanize elderly care to practice age-friendly health care. Fourth, the dilemma and strategy of the case study hospital to achieve AFH certification: 1.Dilemma: (1)There was no grading and difference in the certification standards, (2)Certification standards were not simplified,(3)Most older people lacked health awareness.2.Strategy:(1)Constitute a hospital-level committee and execute care processes in accordance with certification standards,(2)Facilitate the elderly to attend the health education course, use the outpatient waiting to broadcast the video of the health education, and conduct skills assessments, tracking, and referral of the health education center,(3)Build a website to simplify data preparation and facilitate queries. Lastly, presenting the conclusion and suggestions for related units to carry out age-friendly health service hospital proposal.
