摘要: 護理之家為長期照護體系之一環,合格立案之機構每年接受地方衛生機關「督導考核」,每隔三年中央機關內政部衛生署進行「評鑑」,以所獲得之等第作為機構立足於長照體系中服務與品質的象徵,並為社會福利補助評核的給付準則;近年來由於護理人員執業率下降、照顧服務員流動率增加、住民消費意識提升,以及政府政策法規面上機動增修等,在在考驗機構經營團隊營運管理能力,如何在眾多危機中綰合政策、配合政令,以臨床實證提出可行之倡議,始能帶領機構航向醫療照護產業之藍海。
本研究旨在分析護理之家如何能在有限時間內面對上揭經營所需面對的相關課題外,機構領航者C.E.O.(Chief Executive Officer)能具體解決「股東團隊易手」、「負責人變更」、「衛生署護理機構評鑑」,以及「護理之家擴床」等問題,倘以結果論觀之;亦即獲得「成功」,其致勝之關鍵成功因素應值得從事長照機構經營者關注。
本研究為個案研究,查證於文獻,以直接觀察法、部份參與觀察法、深度訪談法及焦點座談法蒐集相關資料後,得其關鍵成功因素有九:1、明確的目標管理;2、正向的政策方針;3、制度化的內部流程;4、可信賴的服務品質;5、有計畫的財務挹注;6、多元化的資源連結;7、可依循的學習成長;8、便利的交通與環境;9、連續多層級的服務輸送。
台灣於國民健康照護組織再造工程已通過《衛生福利部組織法》草案,衛政與社政系統將統合由衛生福利部統籌規劃管理,護理之家所面對的政策法規與現實經營將更為嚴峻,宋?唐庚《文錄》引唐人詩:「山僧不解數甲子,一葉落知天下秋」,願以此例之關鍵成功因素提供予機構經營者卓參為用
Nursing home is a wreath of long-term care system, and the qualified registered ones are under local health authorities' supervision annually and evaluation every three years. The evaluation from the Ministry of the Interior Department of Health is the base for the rankingin the system of long term care services, a symbol of quality and the standard of social welfare grants to assess the benefits; recent years due to decrease of the nurses practicing rate, care attendant increase in turnover, residents consumer awareness, as well as the motor upgrading of government policies and regulations and so on, those problems are the test of team operating management capabilities for the institutions. They have to match up government policies and decree, provide a viable initiative with the clinical evidence, in order to lead agency heading the healthcare industry.
The purpose of this study is to analyze how the nursing homes in limited time on the mortgage business need to face issues as mentioned, and furthermore institutional leader CEO specifically address “the shareholders teamsubstitution"," the person in charge to change, "Department of Health care institutions evaluation", and "expansion of nursing home beds” and other issues. from the consequentialist perspective,to get " success ", the key factors should be worth to engage in long-term care facilities operators attention, including 1) clear goal management; 2)positive policy approach;3)institutionalized internal processes; 4)reliable service quality; 5)inject the financial plan;6)diversity links to resources; 7) can be followed to learn and grow; 8) convenient transportation and the environment; 9)continuity of service delivery.
Government organizations in the national health care re-engineering has passed the draft of the Ministry of Health and Welfare Organization Act, Health and Social governance system integration policy ofare planned and managedt by the Ministry of Health and Welfare, as nursing home care systemwill face the more severe regulations and the business will be more difficult to run: In this study, a case-by-case departure, they face the challenge of the time limit?, are among the practitioners of this work is difficult to escape the plight of, as a Chinese poem said : "Shanseng puzzling number six decades, a Yela known world autumn" cited in Song Dynasty Tang Geng "the text recorded", willing to take cases of critical Key Factors for Successful to institutional operators excellence parameters.
